How is the file-system navigation pane in the "All Navigation" report created?

"All Navigation" displays all the pages for which Google Analytics has recorded pageviews. (That it shows it in a directory structure is just a convenience.) Each page will then show the top 10 recorded entrance and exit points for the page (including back-button activity, etc.).
